Choose the Job Request From (Adobe Acrobat format) that suits your method of billing preference.

Please bring the completed JR Form when submitting your project to the MC-78 Machine Shop along with your sketches, drawings or components to be modified, fitted or repaired.

Sketches and/or drawings should be provided on separate media. (ie. paper, floppy disk, email)

Lengthy consultations will require a completed JR Form. Please make an appointment for Consultation services.
